Wellness Checks

Buckeye police volunteers call the enrolled senior resident at their preferred time and day once a week. After three missed calls in a single day, volunteers will call the senior’s emergency contact to gather more information. If the senior’s wellbeing is not verified, volunteers will visit the home to attempt contact. If further assistance is needed, a police officer will respond and may gain entry to the home.

Eligibility

Buckeye Cares is a voluntary program for seniors who live alone or are homebound, reside within Buckeye and successfully complete enrollment.

Enrollment

A senior resident who wishes to enroll in Buckeye Cares must complete a registration form providing information about their household, the best time to contact them for weekly wellness checks and agree to the installation of a lock box on their property to store keys to their home. The senior resident has the option to provide additional information about medical conditions that may impact their overall health and wellbeing.

Completed registration forms can be emailed to program manager Karlie Conger at kconger@buckeyeaz.gov or dropped off at the Buckeye Police Department (21699 W. Yuma Rd., Suite 104) any time Monday through Friday between 8 a.m. and 5 p.m.

For questions, call Karlie Conger at 623-349-6435.
