Medical Insurance
Buckeye offers eligible employees a choice of three medical coverage options through UnitedHealthcare:
- Choice 500/1000 (in-network only)
- Choice Plus 500/1000 (in- and out-of-network)
- Choice Plus HDHP with HSA 2000/4000 (in- and out-of-network)
All plans include coverage for:
- Emergency care
- Medical services in a doctor’s office
- Hospital care
Dental Insurance
Dental coverage is provided by Delta Dental, with two plan options:
Highlights:
- The city shares the cost of premiums with employees, reducing out-of-pocket expenses
- Using an in-network provider offers the greatest savings and requires no additional paperwork
- You may use an out-of-network provider, but costs may be higher and claims may need to be filed manually
Vision Insurance
Vision coverage is provided by VSP, giving employees access to a large network of providers, including:
- Visionworks
- Walmart, Sam’s Club, Pearle Vision and Costco
- Private practice doctors
Additional features:
- The VSP Premier Edge program offers extra value at Visionworks and select providers
- Employees can shop online at Eyeconic.com for glasses and contact lenses using their benefits
- Benefits can also be used for non-prescription eyewear, such as sunglasses or blue-light filtering glasses
